How did it happen or what did you do to trigger it? Can you recreate it?
After a throw to home plate from the outfielder, the catcher froze after catching the ball and did not attempt to tag the runner. He should've had him out by a mile as shown below. Auto fielding was enabled.

How often can the bug be reproduced?

On a close double play attempt, the runner was called safe and the first baseman raised his arms in disgust and motioned as if he was confronting the umpire. The problem is the umpire was behind him (because he ran over to get into a better position to make the call.)

Playing as a closer in Double A. It wouldn't let me sim past or play past June 28th.
How often can the bug be reproduced?
Everytime I tried to. Which was like 8 times.

So, I made a RTTS LH Pitcher. I still can't believe no one has corrected this, but my player card ALWAYS shows him pitching right handed?? That has been dating back a few years in MLB The Show series... oh well, I have thought previously, but no longer...
Now, when I press select in game, I only get to see the CNR and PWR stats next to the huge scouting report list. NO CNL and PWL?? What is the use of seeing the batters stats against Right handed pitching while pitching in RTTS with my LEFT HANDED pitcher??
Also, I was looking for a left handed pitcher with a NASTY 4 seam. I have trained and trained his 4 seam velocity. He is currently at 99! I have worked his arm strength up to 70, while allowing most other stats to progress slowly... Why is my 4 seam fastball never hitting >90mphs!!!?????
I mostly hit 88-89 on the gun?? 
During the game, the animation scouting report, they say my fast ball is 96-101??!! in the scouting section it says, "Nobody's 4-Seamer brings the heat hotter" ??!!! My scouting report for velocity is A? when I press select in game and press square it says 4SFB 99 mph. Some else with same problem created player with exact stats as RTTS SP and he had 94-99 fastballs. why is RTTS pitching so jacked up??!!!

Did nothing to trigger it. I got called for a strikeout on a first pitch check swing most recently and prior on a 2-1 check swing. The base coach cutscene happened in both scenarios.
How often can the bug be reproduced?

Not sure if i can, what happened was, i hit a line drive up the middle, the ball then hits the pitching rubber and goes foul. The play then goes as a foul and i get more swings which isint how it should be

Any time any of my batters hits a ball into the center field "cubby hole" by the bullpen, to the left of the centerfielder (my point of view is standing at home plate and looking towards the centerfielder), neither the centerfielder, nor left fielder, can go and get the ball. The left fielder "gives up" on the ball assuming the centerfield is going into the "cubby hole" to retrieve the ball, and the centerfielder takes a weird angle where he simply runs into the corner of the "cubby hole" and does not turn the corner to field the ball, meanwhile the batter is circling the bases for an inside the ballpark homerun. Also, the commentary seems to "pause" as the action takes place, and then there is a comment about the inside the ballpark homerun by Hudler. In reality, the hit ball should be a double.
How often can the bug be reproduced?
I have been able to reproduce it more than a dozen times, and every time the centerfield takes the same path, runs into the wall, and does not turn the corner to get into the "cubby hole" meanwhile the runner has an inside the ballpark homerun.Comment

I noticed after a few games with my RTTS SP that my point totals seemed low. I took a closer look at one of the results of the game, and the point total is off by 30. A photo is attached and you can do the math. There are no more events to scroll down to, as evidenced by the fact that "Not Covering Base" is the last entry in the list as it always is.
How often can the bug be reproduced?
Not sure, I will start taking a closer look at the totals which I was taking for granted before. This is a pretty big deal since this year's game offers RTTS points for real $$$, yet is quietly deducting them during normal play...Comment

In Dodger stadium the lines don't wear and tear like the rest of the field does. Last year Dodger stadium was the only stadium that didn't have field degradation for some reason but now the infield has it but the lines stay perfectly fine during the entire game.

Franchise,manage mode. The cpu takes control of the game and sometimes chooses to pitch around a hitter.
How did it happen or what did you do to trigger it? Can you recreate it?
Recent example: VS Arizona, runner at second, two out, Reynolds at the playe and Upton on deck. cpu has my pitcher pitch around Reynolds, Upton hits three run homer.
How often can the bug be reproduced?
I don't know how the game is programmed regarding pitching around hitters, but it happened at times last year also. Unfortunately the workaround from last year, which was to select "pitch batter" from the menu, has been removed this year.≡Comment

Pitcher at the plate, runner on first, less than two out, cpu takes control of the human team and makes the pitcher bunt. There is no option to have the pitcher swing away.
How often can the bug be reproduced?

Runner at second, first base not occupied, pitcher at the plate, one out, cpu brings third and first basemen in to guard against the bunt. Not a bunting situation and even if the pitcher swings at the first pitch they don't move back.
