Increase the court size. I think it will lead to less double teams in the post, and less clutter which seems to happen everytime down court.
Ball handlers should not have their back to the basket 90% of the time no matter where they are on the court. It is far from realistic.
Improve the passing. Players jump way to often when passing to a team mate and the reciever jumps up for the ball too much. Fixing this would improve the fastbreaks as well.
Please, Please, Please remove the music in the game. If I hear that Jermaine Dupri song one more time, I'm going to pass out.Comment

I have a much bigger list but here's a couple:
1) Get rid of the points system for salaries, it just doesn't make much sense to me unless i can think about it in american dollars. They might as well say we're paying them in pecos it is so foreign to me.
2)I had a guy with a three point rating of 77 and he still had the awkward shot release and shot 29% from threes on the year, but LeBron with a 64 rating shot 38% from threes. Get rid of the crappy looking release.
3) Fast breakers need to run lanes instead of hanging back behind the arc.
4) The player who inbounds the ball stays out of bounds for entirely to long.
