Few things piss me off as badly as doing that HURRR! Super-Early off-balance swing at some random breaking crap that's two feet out of the zone after giving yourself the 'Don't Swing' pep talk.

being the impacient person i am, i usually press X after a few seconds after i take a pitch, well, sometimes i press it too early and my guy swings at a ball out of the zone, and i'm like, GOD! lol, it sucks, i give myself a face palm alot, lol

This is a classic facepalm moment, but fortunately it never happens anymore. Best to use an example to explain.

Whenever a runner starts on his way to first, I always 'activate' his icon so I can quickly send him to second in the case of a throwing error. Understandably, 95% of the time it remains a single. Sometimes however, I will forget I have 'activated' this runner and press "X" to skip the cut scene, not realise the play is still live. All of a sudden, my runner takes off for second base as I have instructed him to go home.

Stupid mistake but as I said, can't remember the last time it happened!

Also, fielding a ground ball in the infield and throwing to first - without even remembering there is a runner on base and you could have been out of the inning with a DP.

I fell for the fake to 3rd look to 1st move last night, and got Pujols picked off to end the inning.

I get so mad at myself when I swing at a pitch that hits me, and when I know it will be a ball but I swing just for the sake of it, then begin to call the batter a moron.

I've done plenty of the stuff already posted.
But something else that happened to me:the CPU had runners on 2nd and 3rd and the batter hits one to deep center field and instead of using the cutoff man I threw directly home which resulted in a HUGE arching throw that took forever to get there and so the guy on 2nd scored on the sac fly too lol.
