It has to count when the game is just on as well. I know there is multiple times I've left the system on until it shuts down on its own.

350 out of 363 days I played. Longest was 15 hours straight.

*Fortnite - 911 hours (possibly!)

*Black Ops 4 - 384 (but the in-game counter only has me at like 240)

*Madden - 163 - don't think so. I didn't play Madden much

Yes, it definitely counts idle time. Because although I've played it a ton this year and it is still my most played game of 2018 regardless, there's no way I've played 960 hours of The Show '18. There's a portion of that that is the game running on CPU vs. CPU when I was testing it out by letting games run overnight early in the year (and then being idle until morning when I wake up to look over the box score)

This system doesn't differentiate between the time spent holding the controller and the time the game is active.

This must be including idle time or Netflix. No way I played over 2,000 hours of games last year. I could barely play 1-2 hours a day and in January alone it told me I totaled over 300 hours. That doesn't add up.

Still cool to see.

Spider-Man - 45 Hours
Red Dead Redemption 2 - 44 Hours
God of War - 36 Hours

Those look more realistic.

I'd almost be scared to check how many hours I've logged on some of the open world games I play ...

I do remember when I was in high school, I literally logged half my waking hours in an MMO I was into one year.
