Uni packs are the source of the issue based on your messages. The offender in question must delete the game data (not their saves) but the installed disc data and re-load it. This is the only reliable fix for this. I'm 99% sure it's a uni pack issue.

We actually worked our way around the uni pack issue, it kept disconnecting after the coin toss.
