How it works is this, and this is the part that is most unique: we all know (or should know) that the buttons on the dualshock 3 are pressure sensitive. So using button accuracy, you actually control how hard you throw the ball by how hard you initially press the button. The buttons still represent a base on the diamond (classic, where triangle is 2B, or player relative.)
Once you press the button, the throw meter comes up (it's an arc shape for those wondering) and it has a green sweet spot, with yellow on either side, and red at the end. The colors represent the accuracy as you would imagine (Green = accurate, yellow = offline but not necessarily an error, red = way offline, and most likely an error.) One thing you will notice immediately about the meter is that it is different from player to player AND depending on your players fielding/approach angle. You can actually see it changing as the play develops.
Ok, so how do you use it you say? Once the ball is put in play and you are fielding the ball you press the appropriate button (with a appropriate pressure), the meter comes up and you release it when the indicator line is in the sweet spot. Another great feature is, you don't throw the ball until you release the button, so if you feel you are making a bad throw just keep holding the button and the meter will cycle through again. Or if you want to cancel or redirect a throw, just press the button again (cancel) or press another button throw to another base. Keeping the pressure and everything in mind. The cutoff button [L1] is pressure sensitive and metered in the same way too. This method works the same whether you preload or not.
I'm sure my explanation makes it sound 10X more difficult than it is. It is actually very very intuitive. And most of all, fun. I encourage everyone to at least give it a try when the game drops. Of course, all the other throwing methods are still there if you prefer.
