The main DLCs are Sword of Islam, Legacy of Rome, Sunset Invasion, The Republic, and The Old Gods.
Sunset Invasion is a fantasy scenario that has the Aztecs invading Europe, so you could probably avoid that one unless you want to see that happen.
Sword of Islam enables you to play as a Muslim character and various events and features for Muslim characters. Most of the features came with the patch that went out with Sword of Islam, but they were just usable for the AI. If you have any interest in playing as a Muslim character, then you could get it.
Legacy of Rome mostly focuses on the Byzantine Empire and Orthodox religion. lots of events and decisions for them. again, most of the features came with the patch that came with the DLC, except the Rome specific events I believe. Legacy of Rome also added an ability to have a standing army, Retinues. This is one thing that is only possible if you buy this DLC.
The Republic focuses on coastal merchant republics(like Venice and Genoa). ability to make trade post as a republic, lots of events, and other republic features. all the features were included in the patch for The Republic except the ability to play as a republic. so if you have any interest in playing as a republic, you could get it.
The Old Gods focuses on Pagans, most features came with the patch. With the DLC, you get the ability to play as a Pagan character, and the ability to start in the new start date of 867 AD. I believe the new rebellion system also is tied to the DLC and is only there if you get the DLC.
Ruler Designer allows you to edit any ruler in the game when you start a game. Pretty simple. If you want to create your own character, you could get it.
